Who We Are
The SPAN Center Care Coordination's team work with older adults and their families to maintain and improve the quality of life of older adults in our community. Care Coordinators answer questions, perform assessments, find and coordinate resources, help apply for benefits, and more. Think of them as the liaison for older adults in the care network focused on addressing the needs of older adults and caregivers.
Professional Care Coordination Staff:
- Provide information, referral, assessment, and coordination of services to older adults and their families.
- Develop person-centered service plans focused on strengths, preferences, and needs in conjunction with the individual.
- Coordinate resources to help enhance the quality of life.
- Provide guidance for caregivers seeking support.
- Collaborate with other service professionals and community organizations.
A Care Coordinator will complete an assessment of your unique situation and needs. This can be done in the comfort of your home. You and the Care Coordinator develop a written plan to obtain the services you need. Family members or friends may be involved in the planning process. The Care Coordinator will follow up to confirm that these services are satisfactory to meet your needs to the fullest extent possible and may provide advocacy for you as necessary and appropriate.
