Transition Coordination Provider (TCP) is a Virginia Department of Medical Assistance Services-enrolled provider who is responsible for supporting the individual and family/caregiver, as appropriate, with the activities associated with transitioning from an institution to the community using the CCC Plus Home and Community Based Services Waiver.

Non-emergency Medical Transportation (NEMT) service providers are able to transport riders who require a wheelchair lift-equipped vehicle with appropriate protective restraints or a van that is designed for gurney/stretcher transportation, or that has other types of features to meet safety needs. Some providers also offer limited transportation to grocery stores and food banks.
