Care Transitions

Peninsula Agency on Aging (PAA) provides the "Care Transitions" program. This program is offered in joint partnership between a community-based organization and local hospital to provide additional supports for an individual leaving the hospital and returning home. Consult your local hospital to determine if they participate in this program.
