Who We Are
Sentara PACE, a Program of All-Inclusive Care for the Elderly, program with two locations, Sentara PACE – Churchland, located in Portsmouth and Sentara PACE – Virginia Beach located in Virginia Beach recognizes that older people often have medical problems that last for long periods of time and require services from several different sources. Of great concern is the fact that, as many people age, they become increasingly isolated from the support, companionship and resources of the community at large. Sentara PACE is that important link; assuring participants receive the care they need, when they need it.
This program is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible individuals, but who would prefer to remain at home with services coordinated by Sentara PACE.
This facility located in Virginia Beach provides transportation, meals, medical care and socialization for seniors living with their families, who qualify for nursing facility level of care.
