The Care Coordinator will do a free in-home assessment, asking the right questions to determine your elderly loved one’s needs. From that assessment, a care plan can be developed to ensure that your family, your senior loved one, and health care providers are all on the same page regarding home modification, basic safety, health, nutrition, or other services.
