At Trans-Care Home Health, registered nurses (RNs) provide a broad range of nursing services based on a thorough assessment of the client's health care needs and develops an individualized care plan.  Services provided include skin care, medication management and diet nutrition management, focused care on particular diagnosis.  Physician orders and efficient care coordination with each discipline.



Also provided are physical therapy, occupational therapy, speech therapy; medical social workers, certified nurse aide, personal care aide and home health aide.



Licensed practical nurse assists with dressing changes, oxygen administration and medication and catheter care under the supervision of an RN.
