SenniorBridge (formerly Senior Solutions of Richmond and Mature Options) offers geriatric care management services to the greater Richmond area. Professional Geriatric Care Managers (PGCMs) are health and human services specialists who focus on issues related to aging and elder care.

Core services include assessment, care planning, plan implementation, coordination of services, ongoing monitoring, updates to family and documentation.

Assessments include an evaluation of physical, mental health and cognitive status, environmental safety risks, functional status, financial and legal issues.

Referral, integration and coordination of services may include: adult day care, Alzheimer's assistance, assisted living communities, bill pay services, elder law attorneys, continuing care retirement communities, financial planners, hospice, senior housing, nursing homes, personal emergency response systems, rehab facilities, reverse mortgage specialists, movers, and more.
