Nursing Facility

Brookside Rehab and Nursing Center offers skilled nursing rehabilitation for post hospitalization rehab.  We provide in-patient recovery to facilitate the transition between hospital and home. We also provide long term nursing care to individuals that require continuous assistance with activities of daily living. 

Brookside Rehab and Nursing Center offers one of the areas only secured memory care units that allows for freedom of movement within the unit for those that experience wandering or exit-seeking behaviors with Alzheimer’s or other forms of dementia. We also provide respite care for family care givers so they can relax and recharge. 



Brookside Rehab and Nursing Center is a 130 bed dually certified Medicare/Medicaid skilled nursing and rehab facility.



Conditions treated include fractures, joint replacements, cardiac care, diabetes care, pulmonary care, speech impairments, neurological and neuromuscular disorders, amputations, head and trauma injuries, wound care, pain management, spinal injuries, functional gait issues, incontinence management, rehabilitative restorative dining, ostomy care, stroke rehabilitation and more.
