Care Managers can put in place and monitor services like home care, make sure medical appointments are kept, monitor medication compliance, provide assistance with transition to another living arrangement, and more.
Oftentimes families use IONA's professional Care Management services to relieve the stress of caring for an aging parent or relative. Professionally licensed geriatric care managers provide a comprehensive evaluation, a customized care plan based on your situation, liaison with distant family members and medical and legal professionals, review housing options with you and make recommendations for alternative living arrangements, and counsel a family through this phase of life. Care Managers refer to, coordinate, and monitor home care services, assisted transportation, and respite care. IONA Care Managers can even arrange for in-home doctor visits and medical care.
