GeroPartners offers a Geriatric Care Management program. Guidance, assistance, and support for caregivers of older adults and individuals with chronic illnesses. Our Geriatric Care Manager is a specialist in dementia and mental health, and understands the complex needs of aging adults. Additional services include psychological services to help individuals who need support and treatment for grief and loss issues, depression, anxiety, stress management and other concerns.
