Companions and Friendly Visitors
Programs that use "friendly visitors" to call on people who are hospitalized or in another institutional setting, who are homebound or socially isolated due to disability or age or otherwise lack companionship with the objective of helping them to maintain contact with the outside world by reading, talking, listening, writing letters or performing other similar tasks.
Transition Coordination Provider (MFP)
Transition Coordination Provider (TCP) is a Virginia Department of Medical Assistance Services-enrolled provider who is responsible for supporting the individual and family/caregiver, as appropriate, with the activities associated with transitioning from an institution to the community using the CCC Plus Home and Community Based Services Waiver.
