Who We Are
A program of Horizon Behavioral Health, Intermediate Care Facility (ICF-ID) services at the Altavista Home provides intensive residential support at the highest community-based level. Services are similar to those provided in group homes with the addition of professional contractual staff such as Behavioral Psychologists, Dieticians, Education Specialists, etc.

Intake Process
Call for a required appointment. Same day assessments for all ages are available Monday through Friday from 8:30AM-3:00PM. Same day assessments at the Bedford center are available on Mondays only.
