Case Managers help families plan, coordinate and supervise the care for a loved one. The Geriatric Care Manager offers professional expertise to help with long or short term care arrangements. They have training and background in gerontology and related care professions. The Geriatric Care Manager works alongside the client and family to assist with plans to remain at home.
Other services Care Managers perform are assisting with decisions, arranging and monitoring in-home help, assisting in safe transition to home from a hospital or rehabilitation center, conducting care-planning assessments, reviewing financial medical or legal issues and offer referrals to specialists.
There are different levels of case management according to the level of care needed. Care Management includes: Assessment, Education, Home Care, Consultation, Advocacy, Support, Placement and Crisis Intervention.
