Who We Are
ElderTree offers Geriatric Care Management services. Staff can conduct an overall assessment to identify problems and provide short and long-term solutions. Case manager can provide information about the cost, quality and available resources in the community; ensure availability for on-call emergency services; assist with locating the right long-term care facility; act as a liaison to families at a distance, overseeing care and communicating with the family; screen, arrange and monitor in-home help or other services; and review financial, legal or medical issues and offer referrals.
