Who We Are

Lake County Area Agency on Aging (LCAAA) offers Case Management Services.  Case Management Services assesses the needs of the client and recommends services which will help enable the client to live as independently as possible.

Coordinates resources to meet the needs of the most frail elderly or those with little or no caregiver support. This program is responsible for assessing, reassessing, determining eligibility for Title III, private pay, Medicaid Personal Care and Daycare. Also assists seniors with getting medications for those who are uninsured and low income. All agency services start in this department, except Home Health, and Medicaid or private pay transportation.
