Town Hall Meeting Commonwealth Coordinated Care Plus

The Commonwealth Coordinated Care (CCC) program will end on December 31, 2017. All CCC members will be enrolled into CCC Plus. Commonwealth Coordinated Care Plus will hold a Town Hall Meeting on Wednesday, September 13, 2017, from 2:00 - 4:00pm at City Church in Charlottesville. Due to limited space, registration is required. Come learn about the new program called Commonwealth Coordinated Care Plus (CCC Plus). Have your questions answered by representatives from each of the health plans that you can choose from to provide your Medicaid Benefits.
