Advance Care Medical Planning

The Faith to Fate ministry program targets underserved individuals to provide access and obtain free or very low cost life planning legal documents including an Advance Medical Directive. Provided either under Medicare Part B benefit or private pay options. An Advance Medical Directive will let your designated person know your wishes in case of the onset of a serious illness, a sudden injury, incapacity, dementia, or life’s end. 

The document can be prepared over the telephone or over the computer.
