Care Management Services

Services include arrangement and monitoring home services, providing nutritional guidance, advising on insurance choices and benefits, arranging community resources, consulting with family members, liaison with legal issues, assisting with Medicaid application and providing client transportation.

Also available are in-home assessments, counseling families regarding community services available, supervising privately hired caregivers, assisting families with hiring appropriate person's and communicating with the family. Established clients can call often; takes phone calls from families who live out of this time zone. Works closely with a local agency on aging, social services, LCSW, Medical Center social services, and certified hospice RN.

Services can be provided 24/7; voice mail is available and checked at least 4 times during the day.
