Let’s Make a Plan, Together Brochure: A short brochure featuring a step-by-step overview of advance care planning
Because I Love You Guide: A guide to thinking about, talking about, and planning for the future with family and friends (includes other planning topics, such as funeral planning)
Best for individuals who are familiar with advance care planning and want to take action to create a plan:
Advance Care Planning Information Booklet and Guide: An informational booklet about advance care planning, with resources to help guide individuals through initial steps, including reflection questions
Choosing a Healthcare Agent Decision Card: A one pager with essential information about choosing a healthcare agent/decision-maker (also known as a power of attorney for health care decisions)
Individuals who want to create an advance care plan or execute an advance medical directive can make an appointment, free of charge, with one of our certified First Steps advance care planning facilitators. Please contact us to learn more. We are also providing Virtual Advance Care Planning Appointments.
Best for individuals who have taken action to create a plan and need tools to share the plan with others:
Advance Medical Directive Wallet Card: A sheet of wallet card templates that can be used to record essential information about an individual’s advance medical directive and healthcare agent/decision-maker
