Care Network for Seniors

Care Network for Seniors is an easy-to-use, low or no-cost way for senior citizens (ages 60 and over) with multiple needs to receive services in Fairfax County. The Care Network has case managers from different cultures who speak languages other than English. People of non-English speaking cultures are encouraged to apply.

Develops case plans for the evaluation, treatment, or care of seniors who because of age, illness, disability, or other difficulties are unable to arrange services on their own behalf. Provides a complete assessment of needs by a social worker, nurse, and/or mental health counselor. A case manager coordinates the delivery of services, ensures that the services are obtained in accordance with the case plan, and follows up and monitors progress to ensure that services are having an impact on the problem. All aspects of patient care (medical, social, legal, and financial) are coordinated and managed by staff to minimize the stress and confusion families often experience when dealing with multiple service providers.

The building is fully accessible. Public Transportation includes the Fairfax Connector and Metrobus.
