Elder Care at Home offers a Geriatric Care Manager service who specializes in assisting older people and their families with long term care arrangements. 

Following an in-home assessment, our Care Management team links and coordinates assistance from care providers to enable elderly to obtain highest level of independence consistent with their capacity and preference for care. We arrange & monitor home and medical services; assist with correspondence; help read mail/bill; set up transportation; provide nutritional guidance; coordinate home, auto, schedules & monitor medical appointments; arrange community resources; consult with family members; act as liaison for legal issues; assist with housing placement; plan for travel arrangement; assist with moving & packing, etc.
