Care Navigation for Older Adults

Case Management for Older Adults provides an in home comprehensive assessment of the elder individual or couple. Based on the evaluation, care managers plan and prioritize a care program, then link clients to the services they need. The program assesses the elder's medical, functional, personal, and social needs. The main intent of the program is to enable the elder individual to remain safely in the home with the support necessary to maintain quality of life.

The individual's Care Manager will arrange & monitor home services as determined by the assessment plan.  Help may be provided to assist with correspondence; help read mail/bills; set up transportation; plan recreational activities; provide nutritional guidance; coordinate home, auto, yard maintenance; schedule & monitor medical appointments; assist with insurance claims; advise on insurance choices & benefits; arrange community resources; consult with family members; serve as a liaison with legal issues; assist with housing placement; plan for travel arrangements; assist with moving & packing; assist with Medicaid application.
