Care Management Services

Debra Levy Eldercare Geriatric care management services include identifying options, making recommendations for care, arranging and coordinating services. Home care management, medication management, transportation arrangements, legal and financial assistance, assistance with relocation, monitoring of client's well-being and follow-up care.

Coordination of Care: Staff can coordinate all aspects of care, including, for example, escort to medical appointments, medication management, skilled home nursing care, physical therapy, social and recreational activities, adult day care, grocery or meal delivery, transportation, home maintenance, bookkeeping, medical claims coordination, and friendly visiting.

Assistance with Moving: Help is given an individual to choose best rehabilitation center, nursing home, assisted living or senior community.

The Program can recommend and work with physicians, therapists, attorneys and other professionals providing services to senior adults.
