Support Services

Prince William Area Agency on Aging offers support services.  Comprehensive assessment of older adults to identify needs, initiate service plans, make referrals, arrange and coordinate services, and monitor progress.

Program conducts formal intake by phone to assess needs of older adults, including available support, and need for in-home assessment. Reviews health, financial, and self-care needs. Develops care plan with clients and refers clients to services. Provides follow-up visits and monitors services as needed.

Additional services provided: Assisted Living Facility Assessment for Auxiliary grant; Adult Day Care; Homemaker Services; Coordination of Community Resources; Friendly Visiting; Home Delivered Meals; Home Repairs and Maintenance; Nursing Home Pre-Admission Screening for Medicaid Coverage; Personal Care; Telephone Reassurance; Mental Health.

The care manager will assess the client in his/her home and ask questions about lifestyle, sources of income and ability to provide personal care and medication. Together the participant and the care manager will create a plan for care.
