Care Coordination assists older persons at risk of institutional placement to remain in their homes in their own communities. Care Coordinators investigate a person's needs, preferences, and resources and link the person to a full range of appropriate services and supports, using all available funding sources, and monitoring to ensure that services specified in the support plan are being provided. Mountain Empire Older Citizens, under sub-contract from local housing authorities, is also the care coordinator for residents of public housing.
