Who We Are
LOA Area Agency on Aging offers Care Coordination. Care Coordination helps persons over age 60 remain as independent as possible for as long as possible.
An assessment via home visit to is made to determine the needs a client may have, such as, home delivered meals, in-home services, transportation, adult day care, etc. A case manager can arrange for needed services, develop a plan for your independence, get through the complex red tape of government agencies, and monitor the services once they are in place.
