Care Management Services

Golden Pond ElderCare Strategies offers geriatric care management and consulting services to older adults and their families in a professional, ethical, and caring way to promote comfort and safety in whatever setting they call home.

Services may include referrals to legal, financial or other professional services; liaison between seniors and family members at a distance; reassurance and reassessment visits; facilitation and coordination of long term care arrangements; accompany clients to medical appointments; home safety evaluations; act as advocate for client with health care providers; home safety evaluations; arrange and monitor health-related support systems such as adult day care, home health care and personal care services; arrange transportation and errand services; yard and home maintenance; connection to community support programs; daily money management; assistance in planning and overseeing a move and facilitation of the transition to a new location.

After the initial telephone conversation, if the inquirer wishes to go forward with services, an Agreement for services, Fee Schedule, brochure, business care and other pertinent information is sent and a Plan of Service is developed. The Plan of Service is a flexible document that can be modified (per agreement of the client/family member) as the needs of the client change. No waiting list. Meetings are at the convenience of the client.

Special arrangements can be made to cover certain areas of Fauquier County and Clarke County.
