Commonwealth Coordinated Care Plus (CCC Plus) will hold a Town Hall Meeting on Wednesday, October 4, 2017 from 2:00pm until 4:00pm at The Village Center at Warm Hearth. To register call 1-844-374-9159. For accommodations, send and e-mail to cccplus@dmas.virginia.gov at least one week before the meeting. Registration is required; space is limited. There is no fee.
